Irish Soccer Referees Claims Procedure

If you have
suffered an injury while participating in training for your duties with the ISRS
or in the actual refereeing of a match you should notify your local ISRS branch
representative straight away of this within 48 hours.
·
You will
receive a claim form and explanatory letter regarding the claim process within
the next 2 weeks.
·
If you need to
make any sort of a claim you should fully complete the first page of the claim
form yourself and arrange for your doctor to complete the second page of the
claim form. Please note a branch official must sign the form as well.

If you need
physiotherapy treatment you must get a doctor’s referral for physio prior to
attending.
·
If you have a
private healthcare policy you must claim through that first. Any amounts not
covered by the private healthcare policy can then be claimed through the ISRS
insurance. Confirmation, from the private health insurer, of the amounts not
covered must accompany the claim.

If you
have a medical expenses claim send the completed form along with all your
invoices and receipts to O’Driscoll O’Neil,

2. If everything is in order with your claim you will receive a cheque within 10

If you
have a loss of earnings claim send the completed form along with the 6 weekly
wage slips immediately prior to the accident, confirmation from the social
welfare office of your illness benefit entitlement along with relevant
certificates if you are entitled to illness benefit, confirmation from your
employer of any sick pay payable and a copy of your P60 to O’Driscoll O’Neil, 17
Herbert Place, Dublin 2. If everything is in order with your claim you will
receive a cheque within 10 working days.
